Dental Office Assistant


REPORTS TO: Director of Dental Services
POSITION STATUS: Full-Time
SALARY RANGE: $8.50-$10.45/Hour

SUMMARY

Under the direction of the Director of Dental Services, the Dental Office Assistant is responsible for all billing records, patient registration/discharge, medical records, referrals, and clerical activities that support the functions of the medical practice. Other related duties as assigned

Essential Duties and Responsibilities
Registration/Discharge

  • Greets patients in a customer friendly manner and maintains positive, friendly, interpersonal relationships throughout entire visit.
  • Deals with difficult patients in a professional and compassionate manner, in order to, quickly resolve patient concerns. Directs appropriate concerns to Supervisor.
  • Accurately and efficiently performs registration/discharge process by confirming spelling, accurate demographic information, and insurance information. Maintains and updates patient information on the computer system.
  • Works to ensure smooth office flow by supporting entire medical practice to include but not be limited to, cross training in medical records, referrals and any other office function needed to assist office/patient flow.
  • Prepares encounter forms for pick up, ensures information on encounter forms are accurate, completed and resolved.
  • Prepares cash deposit and maintains log, as per policy and procedure.
  • Adheres to Finance Department Policies and Procedures, including but not limited to, accurate compilation of data into computer system, collection of payment for services rendered, and insurance validation.
  • Ensures that messages taken are accurate and complete in accordance with message taking procedure.
  • Assists in ensuring that patient charts are prepared for office staff and providers prior to the patient appointment.
  • Assist in obtaining chart numbers and information to make charts/patient information available for provider review.

Requirements/Qualifications

Dental office experience/training required. Proven exceptional interpersonal and communication skills, and an ability to multi-task is necessary. Proven organizational and time management skills required. Confidentiality in handling patient and organizational information mandatory. PC knowledge, keyboarding skills required. Some understanding of medical terminology preferred. Bi-lingual skills preferred.

REPORTS TO: Medical Services Coordinator
POSITION LOCATION: Daniel S. Snow, MD Health Center/Harborcreek Health Center/Girard SBHC 

STATUS OF POSITION: Per-Diem (As Needed)
SALARY RANGE: $15.00/Hour (Per-Diem Rate)
SUMMARY

The important role of the Licensed Practical Nurse (LPN) is to support the Physician. This position is responsible for prompt and courteous assistance to all patients of CHN. Responsible for the preparation and maintenance of patient’s clinical records. Prepares reviews, evaluates, interprets, and records vital statistics and other data affecting patient care. Coordinates referrals to other health and social service agencies; establishes and maintains a cooperative relationship with patients. Supports the Physician by facilitating patient flow, providing timely telephone and walk-in triage, including but not limited to, requests for lab results, inquiries regarding current condition, chief compliant and communicating all information to attending physician. Supports the primary care office activities of registration, discharge, medical records, phone reception and scheduling as needed.

REQUIREMENTS:
• Graduation from an accredited school of Practical Nursing
• Current license to practice nursing within the Commonwealth of Pennsylvania
• CPR Certification
• Have one (1) year to two (2) years preferred experience in outpatient medical office
• Provide prompt and courteous assistance to all patients of CHN

HOURS OF WORK:
Flexible Work Hours as needed, Monday through Sunday. Hours of work and days may be adjusted to meet operational needs of the department.

MIS Generalist


REPORTS TO: Chief Information Officer
POSITION LOCATION: Daniel S. Snow, MD Health Center
STATUS OF POSITION: Full-Time
SALARY RANGE: $11.00-$14.00/hour

SUMMARY
Under of the Chief Information Officer uses specialized knowledge or skills to perform hardware support and repairs for organization departments. Resolves hardware, software, and communication problems/conflicts/defects and related issues. Must interact with employees, conducts research, recommend and maintain data communications systems. Install, integrate, and configure computer systems and hardware. Research related products and recommend\implement necessary hardware and software network improvements. Perform helpdesk functions and end user education.

REQUIREMENTS
Proven skills in computerized systems, word processing, spreadsheets, database software, UNIX, and Windows OS. Telecommunications experience a plus. Ability to perform advanced complex repairs and upgrades on a wide variety of computers and peripherals. Ability to diagnose the nature and extent of a wide range of computer problems. Knowledge of advanced computer troubleshooting techniques. Ability to simultaneously coordinate and complete multiple projects. Ability to make administrative/procedural decisions and judgments.

Education and/or Experience
Associate Degree from accredited college or technical school or equivalent experience. Related experience in health care facility preferred.

Float Medical Assistant


REPORTS TO: Medical Services Coordinator
POSITION LOCATION: Various Medical Sites (Float)
STATUS OF POSITION: Full Time
SALARY RANGE: $9.50- $10.50/hour

SUMMARY

Medical Assistant is responsible for assisting the clinical function of the office with patient screening, phlebotomy activities, stocking of supplies and equipment. The Medical Assistant is also expected to support office activities of registration, discharge, referrals, medical records, phone reception, and scheduling as needed.

REQUIREMENTS
Medical Assistant Certification from an accredited institution with equivalent experience required. Must be able to give injections, triage patients and have strong, proven assessment skills, and an ability to work closely with a provider. A successful candidate must be comfortable and competent in working with all populations from pediatric to geriatric. A basic knowledge of the computer keyboarding, excellent communication skills, and an ability to multitask are necessary. It is also essential to relate well to a diverse patient population and to exercise tact, courtesy, and professionalism at all times. Discretion is expected in handling confidential information. Reliable transportation to all CHN medical sites is required.

HOURS OF WORK:

Summary
Under the direction of the Quality and Performance Improvement Coordinator, responsible for assisting the clinical function by maintaining accurate and complete patient records. Other duties as assigned.

Primary Duties and Responsibilities
• Ensure the confidentiality and security of all patient files by following company policies and Federal law.
• Purges records.
• Receives, sorts, and distributes mail and other communications.
• Pull paper charts as needed and files charts appropriately.
• Refile any missed filed information discovered in the patient record.
• Organize and prioritize request for medical records, and enter into patient’s PHI log.
• Electronically file all patient correspondence into the patient’s chart.
• Answer phone calls courteously, timely, and accurately process communications.
• Input patient data into electronic chart.
• Reports all safety problems to the Supervisor immediately.
• Crosstrain and help in other departments as needed.
• Perform other duties as assigned.

Medical Assistant/Community Liaison – Ryan White Part C


REPORTS TO: Program Coordinator – Ryan White Part C
POSITION LOCATION: Daniel S. Snow, MD Building
STATUS OF POSITION: Full Time
SALARY: $10.00-$13.00/hour

SUMMARY
Under the direction of the Ryan White HIV/AIDS Part C Program Director, the Medical Assistant and Community Liaison assist with the clinical and office functions of the Ryan White Program.

Primary Duties and Responsibilities
• Adheres to Essential Duties and Responsibilities of the Community Health Net (CHN) Medical Assistant Position.
• Answers patient questions and triage patient calls.
• Screens patients upon arrival which includes vital signs and screening summaries.
• Calls in patient medications per CHN protocol.
• Calls patients to remind them of Ryan White appointments.
• Schedules, logs and tracks patient medical, dental, eye, and referral appointments.
• Schedules outpatient laboratory and diagnostic testing.
• Participates in outreach activities, with a focus on identifying individuals for testing.
• Conducts HIV testing and counseling, which includes anonymous and confidential testing.
• Prepares new patient packets, orders supplies, tracks all community HIV tests and reports to the state department.
• Assists patients with obtaining HIV medication coverage through SPBP (Special Pharmaceutical Benefits Program)
• Reports all HIV positive tests to the Erie County Health Department
• Attends Northwest PA AIDS Planning Coalition meetings.
• Assists with coordination of Quality Improvement (QI) program, to ensure compliance of clinical protocols, by service providers
• Entry of patient data into the clinical management information system, to include but not be limited to, lab tracking and treatment plan updates.
• Acts as a HIV informational resource, for CHN staff and community contacts.
• Performs other job duties as assigned by the Ryan White Program Director.

Requirements/Qualifications
Current Medical Assistant certification from accredited school, or equivalent experience. Ability to use and navigate a PC, medical terminology, excellent communication skills and ability to multi-task required. Confidentiality in handling patient and organizational information mandatory. Ability to work occasional evenings and weekends for outreach efforts.
