Director of Operations


REPORTS TO: Chief Executive Officer
POSITION LOCATION: 1202 State Street

STATUS OF POSITION: Full-Time Exempt

SUMMARY

The Director of Operations is a key member of the management team at Community Health Net and reports directly to the Chief Executive Officer.  Director of Operations is responsible for the overall operations of patient services.

Essential Duties and Responsibilities 

  • Directs the Joint Commission Accreditation Project (Joint Commission) with the goal of maintaining compliance and accreditation by the Joint Commission. Activities in that regard are coordinated with the Chief Financial Officer, Medical Director, Dental Director, and Human Resources.
  • Responsible for the overall direction, coordination, and evaluation of the medical offices.
  • Works with Medical Director to ensure that services are attractive to patients, efficient, financially successful, productive, and compliant with regulations and corporate policy.
  • Develop or revise as necessary, all medical operational policies and manuals in conjunction with the Medical Director and Chief Executive Officer.
  • Develop and document medical clinical procedures, and ensure that activities in medical clinical services are in accordance with these policies.
  • Director level supervision of Medical Services, Medical Referrals, and Medical Records personnel.
  • Coordinates the resolution of patient complaints and resolution of patient concerns.
  • Participates in the revision and submission of all clinical grant related materials.
  • Directs and maintains HRSA Compliance with the goal of maintaining organizational compliance.
  • Perform other duties as assigned by Chief Executive Officer.

Requirements/Qualifications

Bachelor’s Degree required.  A minimum of five (5) years of experience in healthcare office management, preferably in a primary care setting and general familiarity with managed care, quality management principles preferred.  Strong multi-tasking and project management skills required.  Proven experience in quality management and/or accreditation at the leadership level.  Proven ability to work cooperatively with management and line staff to further organizational goals.  Experience with Joint Commission Accreditation process preferred.

Behavioral Health Coordinator


REPORTS TO: Medical Director
POSITION LOCATION: 1202 State Street

STATUS OF POSITION: Full-Time Exempt

SUMMARY

The Behavioral Health Coordinator will be responsible for providing overall management and coordination of integrated care for Community Health Net (CHN) patients with substance use disorder, mental illness, and co-occurring disorders.  This role includes cross-departmental teamwork with regards to continuous quality improvement, data reporting, and patient care plan Coordinator must be capable of fostering patient-centered collaboration among medical and behavioral health clinical providers and staff, pharmacy personnel, enabling services specialists, partner agencies, organizational management, and other stakeholders. Perform other duties as assigned.

Essential Duties and Responsibilities 

  • Provides oversight of patient screening and assessment for common mental health and substance use disorders and provides brief behavioral interventions using evidence-based techniques. Recognizes and responds appropriately to crisis and emergency situations.
  • Provides administrative and operational leadership and direction, including the coordination and monitoring of staff activities (e.g., policy adherence, staffing, compliance with applicable licensing, audit inspection and regulatory standards).
  • Coordinates all referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, mental health specialty care).
  • Facilitates meetings with appropriate internal and external parties to develop timelines, program agendas, and ensure goals and requirements are met.
  • Maintains documentation, collects data and submits reports as required in a timely manner (e.g., screening, assessments, progress notes, medication prescriptions, case management).
  • Conducts and oversees outreach and patient enrollment into expanded health services. Responsible for community development, selecting outreach sites and ensuring staff are in attendance.
  • Oversees and manages quality assurances and consults with Quality Improvement to develop accurate and appropriate metrics, evaluation questions, data collection, analysis and reports.

Requirements/Qualifications

Bachelor’s degree or higher in Healthcare, Human Services or related field.  Minimum three (3) to five (5) years of work experience in a behavioral health or social service setting.  Knowledge of health center operations, coordination of care, integrated service delivery models, as well as legal and ethical issues related to Medication Assisted Treatment, including the Drug Addiction Treatment Act of 2000. Ability to work with individuals with diverse educational, economic, and cultural backgrounds.  Familiarity with brief, structured counseling techniques (e.g. motivational interviewing and behavioral activation). Ability to multi-task with exceptional communication, time management and organizational skills.  Ability to understand and adhere to behavioral health standards and grant guidelines, including support of Medication Assisted Therapy as a critical component to Substance Use Disorder treatment.

General Dentist


REPORTS TO: Chief Dental Officer
STATUS OF POSITION: Full-Time

SUMMARY

Responsible for the care of dental patients. Such care includes correct diagnosis of the present etiology, informing the patient of the existing state of health of his/her oral cavity, followed by a discussion of the various treatment modalities available, including a discussion of what would follow with treatment and without. Based on this information, a tentative treatment plan, which is acceptable to the patient, will be established. The performance of the agreed upon procedures will be at the highest level of quality within the dentist’s capability.

SUMMARY

The important role of the Licensed Practical Nurse (LPN) is to support the Physician.   This position is responsible for prompt and courteous assistance to all patients of CHN.  Responsible for the preparation and maintenance of patient’s clinical records.  Prepares reviews, evaluates, interprets, and records vital statistics and other data affecting patient care. Coordinates referrals to other health and social service agencies; establishes and maintains a cooperative relationship with patients.  Supports the Physician by facilitating patient flow, providing timely telephone and walk-in triage, including but not limited to, requests for lab results, inquiries regarding current condition, chief compliant and communicating all information to attending physician. Supports the primary care office activities of registration, discharge, medical records, phone reception and scheduling as needed.

Requirements

  • Graduation from an accredited school of Practical Nursing
  • Current license to practice nursing within the Commonwealth of Pennsylvania
  • CPR Certification
  • Have one (1) year to two (2) years preferred experience in outpatient medical office
  • Provide prompt and courteous assistance to all patients of CHN

Medical Assistant


REPORTS TO: Medical Services Coordinator
STATUS OF POSITION: Full-Time

SUMMARY

Medical Assistant is responsible for assisting the clinical function of the office with patient screening, stocking of supplies and equipment.  The Medical Assistant is also expected to support office activities of registration, discharge, referrals, medical records, phone reception, and scheduling as needed.

Requirements

Medical Assistant Certification from an accredited institution with equivalent experience required.  Must be able to give injections, triage patients and have strong, proven assessment skills, and an ability to work closely with a provider. A successful candidate must be comfortable and competent in working with all populations from pediatric to geriatric. A basic knowledge of the computer keyboarding, excellent communication skills, and an ability to multitask are necessary.  It is also essential to relate well to a diverse patient population and to exercise tact, courtesy, and professionalism at all times. Discretion is expected in handling confidential information.  Reliable transportation to all CHN medical sites is required.

Medical Office Assistant


REPORTS TO: Front Office Supervisor
POSITION LOCATION: 1202 State Street

SUMMARY

Under the direction of the Front Office Supervisor, the Office Assistant is responsible for all billing records, patient registration/discharge, medical records, referrals, and clerical activities that support the functions of the medical practice. Other related duties as assigned.

Essential Duties and Responsibilities 

  • Greets patients in a customer friendly manner and maintains positive, friendly, interpersonal relationships throughout entire visit.
  • Assists patients in a professional and compassionate manner, in order to, quickly resolve patient concerns.  Directs appropriate concerns to Supervisor.
  • Accurately and efficiently performs registration/discharge process by confirming spelling, accurate demographic information, and insurance information.  Maintains and updates patient information on the computer system.
  • Works to ensure smooth office flow by supporting entire medical practice to include but not be limited to, cross training in medical records, referrals and any other office function needed to assist office/patient flow.
  • Prepares encounter forms for pick up, ensures information on encounter forms are accurate, completed and resolved.
  • Prepares cash deposit and maintains log, as per policy and procedure.
  • Adheres to Finance Department Policies and Procedures, including but not limited to, accurate compilation of data into computer system, collection of payment for services rendered, and insurance validation.
  • Ensures that messages taken are accurate and complete in accordance with message taking procedure.
  • Assists Medical Records Clerk in ensuring that patient charts are prepared for office staff and providers prior to the patient appointment.
  • Assist Medical Records Clerk in obtaining chart numbers and information to make charts/patient information available for provider review.

Requirements/Qualifications

Certified Medical Secretary Certification preferred or equivalent medical office experience/training required.  Proven exceptional interpersonal and communication skills are necessary.  Proven organizational and time management skills required.  Confidentiality in handling patient and organizational information mandatory.  PC knowledge, keyboarding skills required.  Some understanding of medical terminology preferred.  Bi-lingual skills preferred.

Pharmacy Technician

SUMMARY

Under the direction of a licensed Pharmacist, Pharmacy Technician fills medication orders and other health care products for Community Health Net (CHN) patients. Performs routine tasks to help prepare prescribed medication, such as counting tablets and labeling bottles. Also performs administrative duties, such as answering phones, stocking shelves, and operating cash registers.  Refers any questions regarding prescriptions, drug information, or health matters to a Pharmacist. Other related duties as assigned.

Requirements

  • Knowledge and application of arithmetic and algebra, the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar, principles and processes for providing customer and personal services.
  • This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction required.
  • Valid Pennsylvania driver’s license required
  • Must be willing to take direction.
  • Must be responsible, precise, and organized.
  • Knowledge of pharmacological terminology preferred.
  • No record of substance or drug abuse preferred.
  • May be required to work evenings, weekends, and some holidays based upon operational need.
  • GED or high school diploma required.  Pharmacy Technician diploma from accredited college or technical school or equivalent experience preferred.
  • Two years related experience preferred. Ability to use computers and computer systems to set up functions, enter data, or process information also preferred.

Nurse Case Manager - Ryan White Part C


REPORTS TO: Program Director – Ryan White Part C
POSITION LOCATION: Daniel S. Snow, MD Health Center

STATUS OF POSITION: Full-Time Non-Exempt 

SUMMARY

Under the direction of the Ryan White HIV/AIDS Part C Program Director, the Nurse Case Manager coordinates the primary care for Ryan White patients, which includes, but is not limited to, completing the initial history, patient education, linkage to care, and medication and treatment adherence.

Essential Duties and Responsibilities 

  • Adheres to the essential duties and responsibilities of Community Health Net’s Nurse Position.
  • Provide Nursing job duties for Ryan White and Medical Departments.
  • Case Management: A range of patient-centered services that links patients with primary medical care, physiological and other services to insure timely, coordinates access to medically-appropriate health support services, continuity of care, and services to prevent unnecessary hospitalization.
  • Initiates comprehensive assessments of the patient’s needs and personal support systems.
  • Develops comprehensive, individualized service plans for patients.
  • Coordinates services required to implement service plan.
  • Processes positive HIV referrals: processes intake information, conducts initial physical exam, orders initial lab work, schedules referrals, and access needed to community service programs which specialization in HIV Care and Treatment Services.
  • Conducts confidential HIV testing and counseling.
  • Assists with coordination of Quality Improvement (QI) program, to ensure compliance of clinical protocols, by service providers.
  • Assists patients with obtaining HIV medication coverage through SPBP (Special Pharmaceutical Benefits Program).
  • Reports all HIV positive tests to the Erie County Health Department.
  • Educates patients on the prevention of the spread of HIV, how to cope with the physical, social, and psychological aspects of their disease.
  • Educates and supports loved ones and family members of those infected with HIV.
  • Assists with entry of patient data into the clinical management information system.
  • Perform other job duties as assigned by Ryan White Program Director

Education/Experience

Licensed Practical Nurse with a current PA license and a minimum of two (2) years clinical experience, including phlebotomy, is required.  A minimum of one (1) year experience in Case Management, HIV, and/or Drug and Alcohol counseling experience preferred.
