General Dentist


REPORTS TO: Chief Dental Officer
STATUS OF POSITION: Full-Time

SUMMARY

Responsible for the care of dental patients. Such care includes correct diagnosis of the present etiology, informing the patient of the existing state of health of his/her oral cavity, followed by a discussion of the various treatment modalities available, including a discussion of what would follow with treatment and without. Based on this information, a tentative treatment plan, which is acceptable to the patient, will be established. The performance of the agreed upon procedures will be at the highest level of quality within the dentist’s capability.

Requirements/Qualifications

Qualified applicant must be DMD/DDS with a current Pennsylvania Dental License and DEA License. Knowledge of Electronic Dental Records preferred. Ability to provide quality, comprehensive dental care in a compassionate matter.

Dental Hygienist


REPORTS TO: Chief Dental Officer
STATUS OF POSITION: Full-Time

SUMMARY

Responsible for rendering quality patient care and service. Primary duties include performing quality oral prophylaxis, preparing and promoting dental education, maintaining knowledge of proper radiological procedures, keeping accurate patient dental records, an awareness of departmental equipment and supplies in a cost efficient manner, and performing dental laboratory duties within legal guidelines. Other duties as assigned.

Requirements/Qualifications

Associate’s Degree or equivalent from an accredited school of Dental Hygiene required.  Current Pennsylvania Dental Hygienist license.  A minimum of two (2) years related experience preferred.  Ability to interact and relate to a diverse population, strong written and verbal communication skills.  Meets and maintains credentialing and privileging criteria as outlined in the credentialing and privileging policy.

Health Care for the Homeless (HCH) Program Nurse


REPORTS TO: HCH Program Director
POSITION LOCATION: Daniel S. Snow, MD Health Center
STATUS OF POSITION: Full-Time

SUMMARY

Provides quality healthcare services for Community Health Net’s (CHN) Homeless Program patient population by providing a complete health history and initial health screening to accurately determine the patient’s health care and social needs, establishing and maintaining cooperative relationships with patient, shelter’s staff, coordinating referrals to other health and/or social agencies.

Essential Duties and Responsibilities

  • Complies with duties and responsibilities of CHN’s Nurse Job Description.
  • Conduct new patient intakes for HCH Program.
  • Provide case management for HCH Program patients.
  • Coordinates health care and social services by scheduling initial appointments or referrals for medical, dental, OB/GYN, eye exams, HIV and STD testing, etc.
  • Obtains complete health history to include substance abuse, sexual history, and nutritional intake.
  • Provides assistance and referrals for crisis situations and service providers
  • In cooperation with the HCH Provider, promotes, maintains, and restores the well-being of the client in a professional manner.
  • Completes required grant and health care documentation for each patient.
  • Perform other duties as assigned.

Requirements/Qualifications

Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license to practice in Pennsylvania is required.  Proven success with minimal supervision, proven interpersonal skills, and ability to interact with diverse populations.  Ability to make community contacts to meet goals of the program.  Problem solving skills required.  Community Outreach experience preferred. A minimum of two (2) years related experience preferred.

Family Physician provides primary care to all ages of patients.  These services include but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance.
