Patient Transportation Services

Community Health Net offers free rides for patients seeking medical and dental services at any of our CHN office locations. To arrange for a ride, please call: (814) 454-4530, ext. 229. Arrangements for a ride should be made at the same time an appointment is scheduled. Our van accommodates a limited number of patients per day. Rides are only available to and from CHN owned and operated facilities.

Translation Services

Our mission to provide quality healthcare does not stop at english speaking patients. It only begins there! Community Health Net services patients from many different countries, with many different languages. We won't allow language or cultrual differences to become a barrier to healthcare. LECOM Pharmacists

On Site, one on one patient education on medication use and proper doses. When and how our patients should take medications. They also act as an informational resource on drug interactions.

Health Education

Our health educator will assist you in finding additional information to help you gain a better understanding of health related diagnosis. To name a few: we offer free tobacco cessation classes, Diabetic education, and nutritional support either on a one-on-one bases or in group classes. Diabetes Collaborative

Smoking Cessation
