Licensed Practical Nurse


REPORTS TO: Medical Services Coordinator

SUMMARY

The important role of the Licensed Practical Nurse (LPN) is to support the Physician.   This position is responsible for prompt and courteous assistance to all patients of CHN.  Responsible for the preparation and maintenance of patient’s clinical records.  Prepares reviews, evaluates, interprets, and records vital statistics and other data affecting patient care. Coordinates referrals to other health and social service agencies; establishes and maintains a cooperative relationship with patients.  Supports the Physician by facilitating patient flow, providing timely telephone and walk-in triage, including but not limited to, requests for lab results, inquiries regarding current condition, chief compliant and communicating all information to attending physician. Supports the primary care office activities of registration, discharge, medical records, phone reception and scheduling as needed.

Requirements

  • Graduation from an accredited school of Practical Nursing
  • Current license to practice nursing within the Commonwealth of Pennsylvania
  • CPR Certification
  • Have one (1) year to two (2) years preferred experience in outpatient medical office
  • Provide prompt and courteous assistance to all patients of CHN

Float Medical Assistant


REPORTS TO: Medical Services Coordinator

STATUS OF POSITION: Full-Time

SUMMARY

Medical Assistant is responsible for assisting the clinical function of the office with patient screening, stocking of supplies and equipment.  The Medical Assistant is also expected to support office activities of registration, discharge, referrals, medical records, phone reception, and scheduling as needed.

Requirements

Medical Assistant Certification from an accredited institution with equivalent experience required.  Must be able to give injections, triage patients and have strong, proven assessment skills, and an ability to work closely with a provider. A successful candidate must be comfortable and competent in working with all populations from pediatric to geriatric. A basic knowledge of the computer keyboarding, excellent communication skills, and an ability to multitask are necessary.  It is also essential to relate well to a diverse patient population and to exercise tact, courtesy, and professionalism at all times. Discretion is expected in handling confidential information.  Reliable transportation to all CHN medical sites is required.

Hours of Work

Flexible Work Hours, which total a minimum of 40 hours per week, Monday through Saturday.  Hours of work and days may be adjusted to meet operational needs of the department.

Health Care for the Homeless (HCH) Program Nurse


POSITION TITLE: Health Care for the Homeless (HCH) Program Nurse
REPORTS TO: HCH Program Coordinator
POSITION LOCATION: Daniel S. Snow, MD Health Center
STATUS OF POSITION: Full-time

Summary:
Provides quality healthcare services for Community Health Net’s (CHN) Homeless Program patient population by providing a complete health history and initial health screening to accurately determine the patient’s health care and social needs, establishing and maintaining cooperative relationships with patient, shelter’s staff, coordinating referrals to other health and/or social agencies.

Essential Duties and Responsibilities:
• Complies with duties and responsibilities of CHN’s Nurse Job Description.
• Conduct new patient intakes for HCH Program.
• Provide case management for HCH Program patients.
• Coordinates health care and social services by scheduling initial appointments or referrals for medical, dental, OB/GYN, eye exams, HIV and STD testing, etc.
• Obtains complete health history to include substance abuse, sexual history, and nutritional intake.
• Provides assistance and referrals for crisis situations and service providers
• In cooperation with the HCH Provider, promotes, maintains, and restores the well-being of the client in a professional manner.
• Completes required grant and health care documentation for each patient.
• Perform other duties as assigned.

Qualifications/Requirements:
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license to practice in Pennsylvania is required. Proven success with minimal supervision, proven interpersonal skills, and ability to interact with diverse populations. Ability to make community contacts to meet goals of the program. Problem solving skills required. Community Outreach experience preferred. A minimum of two (2) years related experience preferred.

Billing Associate


POSITION TITLE: Billing Associate
REPORTS TO: Controller
DEPARTMENT: Finance
POSITION LOCATION: Daniel S. Snow, MD Health Center
STATUS OF POSITION: Full Time

Summary
The Billing Associate will support all billing and credentialing activities. They will coordinate and communicate with numerous parties (internal and external) including but not limited to patients, third-party payers, clinical teams, patient support staff, finance and Community Health Net’s third-party billing/ credentialing team.

Duties and Responsibilities
• Assists with patient entry and obtaining accurate data for processing claims on a timely basis.
• Provides follow-up on encounter completion and provider tasking.
• Supports all billing activities including coordination with the billing/ credentialing team on deposits, patient receipts, claim processing, encounter, and third-party payer information.
• Supports all credentialing activities including coordination with the billing/ credentialing team and completion of necessary credentialing information for the third-party payers.
• Provides patient support services in accordance with best practices and in support of CHN’s mission.
• Identifies outstanding denial/ payer issues and coordinates resolution with appropriate parties.
• Provides follow-up on outstanding accounts receivable and collection duties as necessary.
• Participation in recurring meetings with the billing/ credentialing team, including coordination and resolution of outstanding items.
• Coordinates reporting needs for the billing/ credentialing team and other parties as necessary.
• Supports the reporting and reconciliation of the Medicare credit balances and the DHS wraparound reports.
• Provides productivity data and reporting as requested.
• Use computers for various applications, such as database management, word and spreadsheet processing.
• Collaborates with operational/ clinical staff on training as necessary on various topics including but not limited to registration/ demographics, encounter completion, Sliding Fee Discount Program, Migrant Farmworker program, Ryan White and Healthcare for Homeless billing.
• Provides coverage for other Finance duties as necessary.
• Perform other job duties as assigned.

Requirements/Qualifications
• High school diploma (or GED equivalent) with two (2) years applicable experience or Associate/ Bachelor’s degree.
• Certification in Billing and Coding preferred.
• Knowledge of word processing and spreadsheet software (i.e. Microsoft Office).
• Demonstrated ability to create reports and spreadsheets.
• Demonstrated ability to respond to inquiries in a tactful and courteous manner.
• Demonstrated ability to perform duties independently with minimal supervision and to exercise proper judgment.
• Understanding of clinical terminology is required.

Medical Office Assistant

POSITION TITLE: Medical Office Assistant

Summary
Under the direction of the Front Office Supervisor, the Office Assistant is responsible for all billing records, patient registration/discharge, medical records, referrals, and clerical activities that support the functions of the medical practice. Other related duties as assigned.

Essential Duties and Responsibilities
• Greets patients in a customer friendly manner and maintains positive, friendly, interpersonal relationships throughout entire visit.
• Assists patients in a professional and compassionate manner, in order to, quickly resolve patient concerns. Directs appropriate concerns to Supervisor.
• Accurately and efficiently performs registration/discharge process by confirming spelling, accurate demographic information, and insurance information. Maintains and updates patient information on the computer system.
• Works to ensure smooth office flow by supporting entire medical practice to include but not be limited to, cross training in medical records, referrals and any other office function needed to assist office/patient flow.
• Prepares encounter forms for pick up, ensures information on encounter forms are accurate, completed and resolved.
• Prepares cash deposit and maintains log, as per policy and procedure.
• Adheres to Finance Department Policies and Procedures, including but not limited to, accurate compilation of data into computer system, collection of payment for services rendered, and insurance validation.
• Ensures that messages taken are accurate and complete in accordance with message taking procedure.
• Assists Medical Records Clerk in ensuring that patient charts are prepared for office staff and providers prior to the patient appointment.
• Assist Medical Records Clerk in obtaining chart numbers and information to make charts/patient information available for provider review.
Requirements/Qualifications
Certified Medical Secretary Certification preferred or equivalent medical office experience/training required. Proven exceptional interpersonal and communication skills are necessary. Proven organizational and time management skills required. Confidentiality in handling patient and organizational information mandatory. PC knowledge, keyboarding skills required. Some understanding of medical terminology preferred. Bi-lingual skills preferred.

Phone operator would place, answer, transfer calls, and take messages when necessary for the Medical Departments

Duties and Responsibilities
• Receive, transfer, and transmit incoming phone calls.
• Make outbound calls to patients when necessary.
• Schedule and cancel appointments for patients.
• Enter appropriate data in NextGen/Dentrix systems.
• Relay or route written or verbal messages.
• Perform other job duties as assigned

Requirements/Qualifications
High school diploma or General Education Diploma (GED). At least one (1) year of experience using multi line telephone system preferred. Possess excellent verbal and written communication skills. Strong work ethic and can work independently. Must be self-motivated and with the ability to multi-task in a fast-paced environment. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must have professional and telephone etiquette.

Medical Services Coordinator

POSITION TITLE: Medical Services Coordinator
REPORTS TO: Director of Operations
STATUS OF POSITION: Full Time Exempt

Summary:
Under the direction of the Director of Operations, the Medical Services Coordinator is responsible for the effective coordination of the clinical staff and designated clinical area. Provides effective leadership, direction, and administration of clinical operations and personnel to ensure compliance with established policies and procedures. Also responsible for direction, management, and operation of infection prevention, including staff education and consultation with the organization’s leadership, CDC, and the county and state health departments. This role will serve as an adviser to the Director of Operations on all matters pertaining to the operations of medical services. Actively participates and takes leadership role in quality assurance and utilization review.

Primary Duties and Responsibilities:
• Ensures that the daily operating medical activities are appropriate by assigning and overseeing employee workload, to maximize staff productivity which promotes a seamless patient flow process which will maximize resources and improve patient satisfaction. In compliance with CHN’s clinical standards for quality care and responsiveness to patient’s needs.
• Maintains effective and responsive staffing for medical operations. Approves staff Flexible Time Off (FTO), gives advance notification to Director of Operations regarding potential staffing issues.
• Assumes accountability for adequate scheduling of staff based on patient volume, provider availability and operational needs. Makes staffing recommendations to Director of Operations, as needed.
• In conjunction with the Director of Operations monitors daily patient volumes and associated workload (productivity, staffing ratios, patient flow, etc.) to meet required organizational productivity ratios.
• Ensures compliance with regulatory and policy issues related to facilities, clinical practice, risk management.
• Maintains a current inventory of clinical supplies.
• Works cooperatively with Front Office Supervisor to ensure accuracy of provider templates and provides the clinical staff with timely updates regarding provider template and schedule changes. In conjunction with Front Office Supervisor, completes weekly staffing schedule.
• Assists with resolution of patient complaints and resolution of patient concerns for the medical practice.
• Assists with development, implementation and documentation of medical departmental policies and procedures, and ensures that clinical activities are in accordance with these policies.
• Has a complete understanding of the organization nursing, medical assistant, registration, discharge, referral and medical records process, to oversee the comprehensive training of clinical employees.
• Provides nursing coverage as needed to support efficient medical operations.
• Works closely and communicates with Director of Operations on all activities related to medical services and promptly gives notification of problems or potential problems.
• Ensures complete and effective orientation including performance evaluation of introductory/probationary period.
• Coordinates and facilitates clinical staff meetings.
• Act as facilitator to identify opportunities to improve CHN. Responds to and documents various issues including personnel complaints, staff issues, conflicts, and opportunities for improvement. Supports and follows the CHN Employee Concern and Resolution Process.
• Participates in Community Health Net committees, as required, and disseminates information appropriately.
• Perform other job duties as assigned.

Infection Control Duties and Responsibilities:
• Conducts an annual facility infection risk assessment.
• Develops a plan for infection surveillance, prevention and control activities base on the facility
risk assessment.
• Maintains an infection surveillance, prevention, and control program based on the organization’s risk assessment and plan, in accordance with state, federal and other reporting requirements.
• Organizes and maintains surveillance data and records.
• Reports surveillance data, findings and analysis to the appropriate committees.
• Develops evidence base infection prevention and control policies and protocols in accordance with regulatory, government and accrediting/licensing agency requirements and recommendations of relevant professional organizations.
• Updates policies and protocols as needed to maintain compliance with regulations, standards, and evidence-based guidelines.
• Monitors and evaluates the efficacy of infection prevention and control strategies.
• Participates in required annual education programs.
• Provides education on hand hygiene, aseptic technique, infection prevention and control strategies,
antiseptics and disinfectants, specific infectious diseases and other related health care issues.
• Develops or obtains educational materials and programs, as needed, for personnel, patients and visitors.

Requirements/Qualifications:
Current Pennsylvania Registered Nurse (RN) or Licensed Practical Nurse (LPN) and CPR certification required. A minimum of five (5) years nursing experience and three (3) to five (5) years supervisory experience in a medical doctor’s office or equivalent experience required. Strong PC knowledge and keyboarding skills also required. Proven excellence in interpersonal relations, problem solving, organizational skills and ability to follow through to completion required. As well as, an ability to handle numerous interruptions and multi-task required. Must be able to work flexible hours, fill-in for absent staff and work weekends. A positive attitude and ability to communicate professionally and courteously with the public. Proven ability to work effectively with providers, Administrative Staff and other community medical providers required. Proven organizational and time management skills required. Confidentiality in handling patient and organizational information mandatory. Ability to step into all clinical/nonclinical positions within the office and function during times of staffing needs. Experience with NextGen or other EHR system is preferred.

Healthcare for Homeless Program Coordinator

POSITION TITLE: Healthcare for Homeless Program Coordinator
REPORTS TO: Director of Operations

Summary:
Under the direction of the Director of Operations, facilitates a comprehensive program of quality health care and social services for Community Health Net’s homeless patients. Program is to enhance the coordinated delivery of medical and social services and contain a plan of action for homeless patients. Works in conjunction with Community Health Net nursing and provider staff, to coordinate services for health care and social needs, provides crisis intervention services as needed, and performs other duties as assigned.

Primary Duties and Responsibilities:
• Oversees the Health Care for Homeless (HCH) program and monitors for compliance.
• Manages, monitors, and evaluates the activities of the Health Care for Homeless staff.
• Works with Director of Operations to identify organizations that can provide additional support, outreach and expansion for the Health Care for Homeless Program. Responsible for solidifying the cooperative relationship through a letter of agreement/understanding.
• Perform direct clinical services when necessary to maintain clinical operations.
• Continue to develop and improve of the HCH operational process, maintain and update HCH department policies/procedures, and quality assurance program documents, safety, infection, and environmental control programs.
• Per approved policy, submits timely documentation to ensure that all billable services are submitted for payment.
• Participate in the revision and submission of all Homeless Health Care grants and related materials.
• Assist in the development and maintenance of protocols/policies to meet accrediting bodies standards.
• Educate homeless patients and shelter staff, on the availability and affordability of Community Health Net’s holistic services and the importance of personal health maintenance.
• Informs Director of Operations of staffing needs to accomplish department goals.
• Completes required grant and health care documentation for each patient.
• Perform other duties as assigned.

Requirements/Qualifications
Associate Degree or equivalent experience required. A minimum of (2) years related experience required. Strong PC knowledge and keyboarding skills also required.
