General Dentist


REPORTS TO: Chief Dental Officer
STATUS OF POSITION: Full-Time

SUMMARY

Responsible for the care of dental patients. Such care includes correct diagnosis of the present etiology, informing the patient of the existing state of health of his/her oral cavity, followed by a discussion of the various treatment modalities available, including a discussion of what would follow with treatment and without. Based on this information, a tentative treatment plan, which is acceptable to the patient, will be established. The performance of the agreed upon procedures will be at the highest level of quality within the dentist’s capability.

SUMMARY

The important role of the Licensed Practical Nurse (LPN) is to support the Physician.   This position is responsible for prompt and courteous assistance to all patients of CHN.  Responsible for the preparation and maintenance of patient’s clinical records.  Prepares reviews, evaluates, interprets, and records vital statistics and other data affecting patient care. Coordinates referrals to other health and social service agencies; establishes and maintains a cooperative relationship with patients.  Supports the Physician by facilitating patient flow, providing timely telephone and walk-in triage, including but not limited to, requests for lab results, inquiries regarding current condition, chief compliant and communicating all information to attending physician. Supports the primary care office activities of registration, discharge, medical records, phone reception and scheduling as needed.

Requirements

  • Graduation from an accredited school of Practical Nursing

Float Medical Assistant

SUMMARY

Medical Assistant is responsible for assisting the clinical function of the office with patient screening, stocking of supplies and equipment.  The Medical Assistant is also expected to support office activities of registration, discharge, referrals, medical records, phone reception, and scheduling as needed.

Requirements

Medical Assistant Certification from an accredited institution with equivalent experience required.  Must be able to give injections, triage patients and have strong, proven assessment skills, and an ability to work closely with a provider. A successful candidate must be comfortable and competent in working with all populations from pediatric to geriatric. A basic knowledge of the computer keyboarding, excellent communication skills, and an ability to multitask are necessary.  It is also essential to relate well to a diverse patient population and to exercise tact, courtesy, and professionalism at all times. Discretion is expected in handling confidential information.  Reliable transportation to all CHN medical sites is required.

Hours of Work

Flexible Work Hours, which total a minimum of 40 hours per week, Monday through Saturday.  Hours of work and days may be adjusted to meet operational needs of the department.

Nurse Case Manager - Ryan White Part C


REPORTS TO: Program Director – Ryan White Part C

SUMMARY

Under the direction of the Ryan White HIV/AIDS Part C Program Director, the Nurse Case Manager coordinates the primary care for Ryan White patients, which includes, but is not limited to, completing the initial history, patient education, linkage to care, and medication and treatment adherence.

Essential Duties and Responsibilities 

  • Adheres to the essential duties and responsibilities of Community Health Net’s Nurse Position.
  • Provide Nursing job duties for Ryan White and Medical Departments.
  • Case Management: A range of patient-centered services that links patients with primary medical care, physiological and other services to insure timely, coordinates access to medically-appropriate health support services, continuity of care, and services to prevent unnecessary hospitalization.
  • Initiates comprehensive assessments of the patient’s needs and personal support systems.
  • Develops comprehensive, individualized service plans for patients.
  • Coordinates services required to implement service plan.
  • Processes positive HIV referrals: processes intake information, conducts initial physical exam, orders initial lab work, schedules referrals, and access needed to community service programs which specialization in HIV Care and Treatment Services.
  • Conducts confidential HIV testing and counseling.
  • Assists with coordination of Quality Improvement (QI) program, to ensure compliance of clinical protocols, by service providers.
  • Assists patients with obtaining HIV medication coverage through SPBP (Special Pharmaceutical Benefits Program).
  • Reports all HIV positive tests to the Erie County Health Department.
  • Educates patients on the prevention of the spread of HIV, how to cope with the physical, social, and psychological aspects of their disease.
  • Educates and supports loved ones and family members of those infected with HIV.
  • Assists with entry of patient data into the clinical management information system.

Education/Experience

Licensed Practical Nurse with a current PA license and a minimum of two (2) years clinical experience, including phlebotomy, is required.  A minimum of one (1) year experience in Case Management, HIV, and/or Drug and Alcohol counseling experience preferred.

Quality Improvement Assistant


POSITION TITLE: Quality Improvement Assistant

Summary
Quality Improvement Assistant is responsible for assisting in carrying out quality improvement processes. This will include but is not limited to recording audit reviews and utilization reviews. Within federal, state, and local guidelines, the Quality Improvement Assistant will assist in performing routing work, including data entry, Electronic Health Record (EHR) review, assisting with administering internal quality assurance and compliance activities, and assisting with quality assurance audits of health outcomes, as defined by the Uniform Data System (UDS), Patient Centered Medical Home (PCMH), and organizational goals.

Essential Duties and Responsibilities

• Provides support for Healthcare Effectiveness Data and Information Set (HEDIS) quality initiatives and regulatory/contractual requirements including outreach to members who are noncompliant in the designated outreach services.
• Supports effective medical record capture for HEDIS medical review and abstraction of supplemental data.
• Provides care coordination for assigned patients by coordinating patient appointments.
• Responsible for completing outreach and subsequent intake activities using required forms and documents.
• Conduct patient telephone outreach and engagement, on-going emerging needs and to promote continuity of care and improved health outcomes.
• Follow up with Hospitals/ER upon notification of admission or discharge and coordinate with health care team to facilitate communication of the event and transition of care services, to include, but not limited to medication reconciliation with health care team as per policy.
• Assist patient in developing plans and community supports to overcome obstacles that would prevent them from receiving needed services/referrals.
• Assist with preparation of evaluation reports
• Develops systematic approaches to identify data errors and service trends.
• Assist with patient satisfaction surveys, including, but not limited to; working directly with patients and staff, compiling results, and entering data.

Qualifications/Requirements
• High school diploma or GED required. Associate degree in Medical or related field preferred.
• Certified Medical Assistant or Licensed Practical Nurse (LPN) with minimum two (2) years’ experience.
• Ability to use computers and Windows based software to enter data and develop reports as needed.
• Excellent writing, communication, and organizational skills.

Summary
Under the direction of the Ryan White HIV/AIDS Part C Program Director, the Medical Assistant and Community Liaison assist with the clinical and office functions of the Ryan White Program.

Primary Duties and Responsibilities
• Adheres to Essential Duties and Responsibilities of the Community Health Net (CHN) Medical Assistant Position.
• Answers patient questions and triage patient calls.
• Screens patients upon arrival which includes vital signs and screening summaries.
• Calls in patient medications per CHN protocol.
• Calls patients to remind them of Ryan White appointments.
• Schedules, logs and tracks patient medical, dental, eye, and referral appointments.
• Schedules outpatient laboratory and diagnostic testing.
• Participates in outreach activities, with a focus on identifying individuals for testing.
• Conducts HIV testing and counseling, which includes anonymous and confidential testing.
• Prepares new patient packets, orders supplies, tracks all community HIV tests and reports to the state department.

Essential Duties and Responsibilities:
• Complies with duties and responsibilities of CHN’s Nurse Job Description.
• Conduct new patient intakes for HCH Program.
• Provide case management for HCH Program patients.
• Coordinates health care and social services by scheduling initial appointments or referrals for medical, dental, OB/GYN, eye exams, HIV and STD testing, etc.
• Obtains complete health history to include substance abuse, sexual history, and nutritional intake.
• Provides assistance and referrals for crisis situations and service providers
• In cooperation with the HCH Provider, promotes, maintains, and restores the well-being of the client in a professional manner.
• Completes required grant and health care documentation for each patient.

Qualifications/Requirements:
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license to practice in Pennsylvania is required. Proven success with minimal supervision, proven interpersonal skills, and ability to interact with diverse populations. Ability to make community contacts to meet goals of the program. Problem solving skills required. Community Outreach experience preferred. A minimum of two (2) years related experience preferred.

Billing Associate


POSITION TITLE: Billing Associate
REPORTS TO: Controller

Summary
The Billing Associate will support all billing and credentialing activities. They will coordinate and communicate with numerous parties (internal and external) including but not limited to patients, third-party payers, clinical teams, patient support staff, finance and Community Health Net’s third-party billing/ credentialing team.

Duties and Responsibilities
• Assists with patient entry and obtaining accurate data for processing claims on a timely basis.
• Provides follow-up on encounter completion and provider tasking.
• Supports all billing activities including coordination with the billing/ credentialing team on deposits, patient receipts, claim processing, encounter, and third-party payer information.
• Supports all credentialing activities including coordination with the billing/ credentialing team and completion of necessary credentialing information for the third-party payers.
• Provides patient support services in accordance with best practices and in support of CHN’s mission.
• Identifies outstanding denial/ payer issues and coordinates resolution with appropriate parties.
• Provides follow-up on outstanding accounts receivable and collection duties as necessary.
• Participation in recurring meetings with the billing/ credentialing team, including coordination and resolution of outstanding items.
• Coordinates reporting needs for the billing/ credentialing team and other parties as necessary.
• Supports the reporting and reconciliation of the Medicare credit balances and the DHS wraparound reports.
• Provides productivity data and reporting as requested.
• Use computers for various applications, such as database management, word and spreadsheet processing.
• Collaborates with operational/ clinical staff on training as necessary on various topics including but not limited to registration/ demographics, encounter completion, Sliding Fee Discount Program, Migrant Farmworker program, Ryan White and Healthcare for Homeless billing.
• Provides coverage for other Finance duties as necessary.

Requirements/Qualifications
• High school diploma (or GED equivalent) with two (2) years applicable experience or Associate/ Bachelor’s degree.
• Certification in Billing and Coding preferred.
• Knowledge of word processing and spreadsheet software (i.e. Microsoft Office).
• Demonstrated ability to create reports and spreadsheets.
• Demonstrated ability to respond to inquiries in a tactful and courteous manner.

Summary:

The Program Director oversees all aspects of the Ryan White HIV/AIDS (RWHAP) Part C Program in collaboration with the RWHAP Part C Program Director, Clinician, and Ryan White Program Director.

Job duties include:

• Provides planning and support services for the Community Health Net (CHN) Ryan White Part C program in collaboration with the clinical staff.
• Facilitates meetings with appropriate CHN staff, and external sources, to develop timelines, program agendas, and to ensure that program goals and HRSA requirements are met.
• Liaison to funding source, key partners and other key stakeholders.
• Coordinates and prepares reports, evaluates program data, assists with funding stream analysis and needs assessment report, and participates in the drafting of proposals and other reports for HRSA.
• Works cooperatively with Finance Department with monitoring the Ryan White budget to coordinate fiscal administration, in accordance with the grant regulations.
• Conducts and oversees outreach. Responsible for selecting outreach sites and ensuring staff are in attendance.
• Develops, selects and distributes communication/public relations brochures and educational materials. Responsible for ensuring such information is distributed to CHN Sites and affiliate organizations.
• Liaison with nursing staff, provider staff and community agencies to facilitate the problem-solving process for Ryan White Program initiatives Coordinates CHN Ryan White healthcare services and provide educational resources.
• Maintains and writes department policies, procedures, and objectives.
• Oversees and manages quality assurances program, infection and environmental control for Ryan White program. Collects relative data and generates reports to funding source and others as appropriate.
• Attends Northwest PA AIDS Planning Coalition meetings.
• Orients new Ryan White staff, program and clinical visitors on Ryan White Program.
• Coordinates Community Health Net, Ryan White staff education.
• Performs other job duties as assigned.

Qualifications include:

• Bachelor’s degree in nursing, social work, public/business administration, health or related field and one year of case management experience, or any combination of education, training and experience which demonstrates the ability to perform the duties of the position required.
• Must be able to communicate effectively verbally and in writing, prepare complete and accurate reports and records, and network with community resource agencies.
• A minimum of two (2) years supervisory experience, experience in community networking, grant management, and a working knowledge of the challenges and needs of the HIV population and existing HIV/AIDS resources preferred.

Summary:

Provides consultation to primary care team, including the patients’ medical, dental, and psychiatric providers. Responsible for supporting and coordinating the mental and physical health care of Community Health Net patients’ medical provider and consulting psychiatrist.

Job duties include:

• Perform Behavioral Health consultation for primary care providers by providing brief behavioral interventions using evidence-based techniques.
• Conduct screenings and assessment of Behavioral Health/Substance Abuse patients.
• Coordinate patient care and refer to external providers for additional treatment as needed.
• Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
• Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
• Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.
• Collaborate with patients’ medical provider to develop individualized treatment plans.
• Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider.
• Triage patients to determine where their needs can be best met.
• Document and update patient’s chart in electronic health record as per Community Health Net Clinical Policies and Procedures.
• Meet productivity and quality standards set forth by Community Health Net.

Education/Development
Required
• Master’s Degree in Healthcare, Human Services or related field.
• Valid and current Pennsylvania (PA) License as a: LCSW, LPC, and LMFT
• Minimum of one (1) year of work experience in a behavioral health or social service setting.

• Experience working with underserved patient populations and safety net providers.
• Experience working with Pennsylvania and Federal programs.

Summary:

Behavioral Health Assistant performs certain administrative and clinical duties under the direction of Community Health Net’s Behavioral Health Coordinator. Administrative duties may include registration, discharge, scheduling, faxing, and answering phones, as needed. Clinical duties may include taking and recording vital signs and medical histories, preparing patients for examination, and assist Medical Providers/Behavioral Health Consultants as needed.

Job duties include:

• Record patients’ medical history, vital statistics, or other health information in Community Health Net’s Electronic Health Record (EHR).
• Assess physical conditions, mental health, and any substance abuse disorder of patients to aid in diagnosis or treatment.
• Provide brief assessment and intervention for patients with behavioral health and substance abuse disorders.
• Administers injections, immunizations, and associated quality controls as directed by Community Health Net Medical Provider.
• Performs testing procedures such as but not limited to; patient urine dipstick, rapid strep, fecal occult blood test, and urine pregnancy test.
• Immediately report abnormal testing procedure results to appropriate medical personnel.
• Assist Nurses, Medical Providers, and Behavioral Health Consultants with medical and behavioral health care as needed.
• Serve as a backup for Behavioral Health Consultants.
• Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.
• Participate in treatment plan development, crisis management, and patient education.
• Assist with managing registry of behavioral health and substance abuse patients.
• Dispose of biomedical waste in accordance with clinical standards.
• Prepare, sterilize, and clean patient treatment rooms for use.
• Report all patient/safety concerns and near misses in event reporting, printing and notifying supervisor at time of report.

Qualifications include:

Summary
Executes patient insurance referrals, consultations, and diagnostic procedures relating to the healthcare facility.

Duties and Responsibilities
• Schedules consultations and diagnostic appointments with hospitals and specialty offices for patients, per provider request, and documents necessary information.
• Accurately and timely completes requested forms for insurance and specialty offices.
• Obtains insurance prior authorization for appointments.
• Contacts patients to provide appointment date, time, location and preparation information for appointment, if appropriate.
• Answers questions and conducts research to answer questions regarding patient referral appointments and testing.
• Answers telephone, screens calls, takes messages, and provides information regarding patient referrals.
• Works closely with other internal departments to assure all the necessary patient information is received to complete the patient referral.
• Reschedules missed appointments as necessary.
• Assures referrals are properly tracked and documented as to no show, cancelled, and rescheduled appointments.
• Reviews encounters for changed insurance information and changing demographics, as needed.

Requirements/Qualifications

Pharmacy Technician

POSITION TITLE: Pharmacy Technician

SUMMARY
Under the direction of a licensed Pharmacist fills medication orders and other health care products for Community Health Net (CHN) patients. Performs routine tasks to help prepare prescribed medication, such as counting tablets and labeling bottles. Also performs administrative duties, such as answering phones, stocking shelves, and operating cash registers. Refers any questions regarding prescriptions, drug information, or health matters to a Pharmacist.

Essential Duties and Responsibilities
• Receive and process written, electronic, or faxed prescription orders and verify that information is complete and accurate.
• Accurately retrieve demographic, insurance, medication allergy and CHN program eligibility information from Community Health Net’s Electronic Health Record (EHR) as needed.
• Maintain proper storage and security conditions for drugs.
• Fill bottles/vials with prescribed medications and affix labels.
• Under the guidance of a Registered Pharmacist, reconstitute pharmaceutical products according to written prescriptions.
• File and retrieve hard-copy prescriptions that have been processed.
• Clean and help maintain equipment and work areas.
• Establish and maintain patient profiles, including lists of medications taken by individual patients.
• Receive and store incoming supplies, verify quantities against invoices, and inform supervisors of stock needs and shortages as directed.
• Answer telephones and responds to questions or requests.
• Operate cash registers to accept payment from customers.
• Assist customers by answering simple questions, locating items or referring them to the pharmacist for medication information or counseling.
• Perform other job-related duties as directed.
