The mission of Integrated Behavioral Health at Community Health Net is to improve the mental wellness of our region through supportive, integrated, patient-centered care that is compassionate, dignified, informative, and individualized.

Mental Health

We offer treatment for depression and anxiety, providing a bridge to specialty community mental health services.

Care Management

We provide case management for patients to assist with access and coordination of community services.

Addiction Services

We provide medication-assisted treatment for Opioid and Alcohol Use Disorder, with therapeutic intervention to support recovery.

• A licensed Behavioral Health Counselor is available to our patients at the time of medical visit.

• Individual therapy available by licensed counselors.

• Care Coordination will provide linkage to services in the community to improve our quality of life.
