Pennsylvania COVID-19 Vaccination Phases

Federally qualified health centers have agreed to vaccinate under guidance from the federal government and the state departments of health.

Community Health Net, the leading Federally Qualified Health Center (FQHC) serving the Lake Erie region, is seeking to ensure the highest level of protection from COVID-19 for the Erie Community by offering scheduled appointments for vaccinations of the Moderna COVID-19 Vaccine to all people above the age of 18.

All individuals , 18 years of age and older, are eligible for the Moderna COVID-19 vaccine. The Moderna vaccine is administered in two doses. First doses are able to be scheduled by clicking here to book your appointment. All second doses will be scheduled at first dose appointment between the recommended window of 28 – 42 days.

Community Health Net is currently offering Moderna 3rd Dose/Boosters for those who are moderately to severely immunocompromised. Booster vaccine appointments can be scheduled by clicking here and selecting the 3rd Dose/Booster option. Indvidual’s who are not immunocompromised are recommended to recieve the booster at the 8th month mark from completing their initial Moderna COVID-19 vaccine series.
