Pennsylvania COVID-19 Vaccination Phases
Federally qualified health centers have agreed to vaccinate under guidance from the federal government and the state departments of health. The Pennsylvania Department of Health has recommended vaccine priorities for Phase 1C.
Community Health Net, the leading Federally Qualified Health Center (FQHC) serving the Lake Erie region, is seeking to ensure the highest level of protection from COVID-19 for the Erie Community by offering scheduled injections of the Moderna COVID-19 Vaccine to healthcare professionals in health-related agencies or private practices.
In compliance with Federal standards, we will schedule you in accordance with your risk phase. The Moderna vaccine is administered in two doses. As vaccines for your risk phase are received, we will notify you to schedule the first injection. The second dose will be scheduled 28 days after the initial injection. We recommend that, if you are registering your office, you do not plan to vaccinate your entire staff simultaneously, as the potential for adverse reactions could result in office closure.
Please note that Community Health Net must consider the most efficient use of the vaccine supply while remaining as flexible as possible when scheduling personnel. Your patience is appreciated.
