POSITION TITLE: Referrals Specialist
REPORTS TO: Quality and Performance Improvement Coordinator
STATUS OF POSITION: Full-Time 

Summary

Executes patient insurance referrals, consultations, and diagnostic procedures relating to the healthcare facility.

Essential Duties and Responsibilities

  • Schedules consultations and diagnostic appointments with hospitals and specialty offices for patients, per provider request, and documents necessary information.
  • Accurately and timely completes requested forms for insurance and specialty offices.
  • Obtains insurance prior authorization for appointments.
  • Contacts patients to provide appointment date, time, location and preparation information for appointment, if appropriate.
  • Answers questions and conducts research to answer questions regarding patient referral appointments and testing.
  • Answers telephone, screens calls, takes messages, and provides information regarding patient referrals.
  • Works closely with other internal departments to assure all the necessary patient information is received to complete the patient referral.
  • Reschedules missed appointments as necessary.
  • Assures referrals are properly tracked and documented as to no show, cancelled, and rescheduled appointments.
  • Reviews encounters for changed insurance information and changing demographics, as needed.
  • Perform other job duties as assigned.

Education/Experience

High School Diploma or GED required.  Medical Assistant Certification or equivalent experience preferred. Two (2) years related experience preferred.  Proficient in word processing, data processing, data input, and searching for information on computer applications.  Strong organizational skills, prioritization skills and a systematic approach to work with a high level of accuracy and attention to detail.  Able to work independently and act on own initiative.  Ability to read, write and converse fluently in English. Understanding of medical terminology.

POSITION TITLE: Psychiatric Nurse Practitioner
REPORTS TO: Medical Director
STATUS OF POSITION: Full Time

Summary

Psychiatric Nurse Practitioner provides primary care to Behavioral Health patients within an integrated care model. These services include but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance.

Essential Functions:

  • Practices under the standard procedure established for the Nurse Practitioner role consistent with Community Health Net policy and in accordance with requirement set forth by the Pennsylvania Board of Nursing.
  • Conducts age appropriate assessments of patients, including history taking and physical examination consistent with department protocol.
  • Performs assessment/data collection in an on-going systematic manner, focusing on physiologic, psychosocial, cultural, spiritual and cognitive status, patient/family education, family involvement, and patient advocacy.
  • Consults with supervising physician or other clinicians as dictated by patient need and assessment findings.
  • Participates with physicians and other clinicians in the department to assure effective patient care consistent with quality indicators for patients cared for in the designated service.
  • Reviews medical record for follow up or ordering of diagnostic studies under established protocols.
  • Establishes and maintains communication with other health care providers and departments to ensure operational efficiency and effectiveness and to promote consistency and continuity of care.
  • Demonstrates a service philosophy in all interactions with patients, families, and all members of the health care team.
  • Demonstrates proficiency in the use of computers and telecommunication modalities to document, track and convey information.

Requirements/Qualifications

Qualified applicant must possess a valid and current Pennsylvania Nurse Practitioner and Registered Nurse License. Master’s degree in Nursing. Completion of master’s program in Adult NP, Family Practice NP, Acute Care NP or Psych/Mental Health NP required. One (1) year of experience as a Nurse Practitioner in an inpatient or outpatient psychiatric care facility, including the provision of treatment and crisis intervention for members with acute or chronic psychiatric disorders. Current DEA License preferred. Must successfully complete the required credentialing process. Must have an NPI and meet Medicare requirements for PTAN (Provider Transaction Account Number). Knowledge of Electronic Health Records preferred. Ability to provide quality, comprehensive medical care in a compassionate matter.

SUMMARY

Responsible for the delivery of quality healthcare for clients of Community Health Net. Primary duties include providing a complete health history and accurately determining client’s medical condition. In cooperation with the medical provider, promotes, maintains, and restores the well- being of the patient in a professional manner. Coordinates referrals to other health and social agencies; establishing and maintaining cooperative relationships with clients, families, providers, and coworkers. Maintains an awareness of medical equipment and supplies in a cost effective and safe manner. All nursing duties assigned are pending training and successfully passing competencies. Supports office activities of registration, discharge, medical records, and scheduling, as needed.

SUMMARY

Medical Assistant performs certain administrative and clinical duties under the direction of Community Health Net’s Medical Services Coordinator. Administrative duties may include registration, discharge, scheduling, faxing, and answering phones, as needed. Clinical duties may include taking and recording vital signs and medical histories, preparing patients for examination, and assist Medical Providers as needed.

Essential Duties and Responsibilities

  • Record patients’ medical history, vital statistics, or other health information in Community Health Net’s Electronic Health Record (EHR).
  • Assess physical conditions of patients to aid diagnosis or treatment.
  • Administers injections, immunizations, and associated quality controls as directed by Community Health Net Medical Provider.
  • Performs testing procedures such as but not limited to; patient urine dipstick, rapid strep, fecal occult blood test, and urine pregnancy test.
  • Immediately report abnormal testing procedure results to appropriate medical personnel.
  • Assist Nurses and Medical Providers with medical procedures as needed.
  • Dispose of biomedical waste in accordance with clinical standards.
  • Prepare, sterilize, and clean patient treatment rooms for use.

POSITION TITLE: Health Care for the Homeless (HCH) Program Nurse
REPORTS TO: HCH Program Coordinator

Summary:

Provides quality healthcare services for Community Health Net’s (CHN) Homeless Program patient population by providing a complete health history and initial health screening to accurately determine the patient’s health care and social needs, establishing and maintaining cooperative relationships with patient, shelter’s staff, coordinating referrals to other health and/or social agencies.

Essential Duties and Responsibilities:

  • Complies with duties and responsibilities of CHN’s Nurse Job Description.
  • Conduct new patient intakes for HCH Program.
  • Provide case management for HCH Program patients.
  • Coordinates health care and social services by scheduling initial appointments or referrals for medical, dental, OB/GYN, eye exams, HIV and STD testing, etc.
  • Obtains complete health history to include substance abuse, sexual history, and nutritional intake.
  • Provides assistance and referrals for crisis situations and service providers
  • In cooperation with the HCH Provider, promotes, maintains, and restores the well-being of the client in a professional manner.
  • Completes required grant and health care documentation for each patient.

Qualifications/Requirements:

Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license to practice in Pennsylvania is required. Proven success with minimal supervision, proven interpersonal skills, and ability to interact with diverse populations. Ability to make community contacts to meet goals of the program. Problem solving skills required. Community Outreach experience preferred. A minimum of two (2) years related experience preferred.

POSITION TITLE: Medical Office Assistant
REPORTS TO:Front Office Supervisor

Summary

Under the direction of the Front Office Supervisor, the Office Assistant is responsible for all billing records, patient registration/discharge, medical records, referrals, and clerical activities that support the functions of the medical practice. Other related duties as assigned.

Essential Duties and Responsibilities

  • Greets patients in a customer friendly manner and maintains positive, friendly, interpersonal relationships throughout entire visit.
  • Assists patients in a professional and compassionate manner, in order to, quickly resolve patient concerns. Directs appropriate concerns to Supervisor.
  • Accurately and efficiently performs registration/discharge process by confirming spelling, accurate demographic information, and insurance information.
  • Maintains and updates patient information on the computer system.
  • Works to ensure smooth office flow by supporting entire medical practice to include but not be limited to, cross training in medical records, referrals and any other office function needed to assist office/patient flow.
  • Prepares encounter forms for pick up, ensures information on encounter forms are accurate, completed and resolved.

Requirements/Qualifications

Certified Medical Secretary Certification preferred or equivalent medical office experience/training required. Proven exceptional interpersonal and communication skills are necessary. POSITION TITLE: Medical Services Coordinator
REPORTS TO: Director of Operations
STATUS OF POSITION: Full Time Exempt

Summary

Under the direction of the Director of Operations, the Medical Services Coordinator is responsible for the effective coordination of the clinical staff and designated clinical area. Provides effective leadership, direction, and administration of clinical operations and personnel to ensure compliance with established policies and procedures. Also responsible for the direction, management, and operation of infection prevention, including staff education and consultation with the organization’s leadership, CDC, and the county and state health departments. This role will serve as an adviser to the Director of Operations on all matters pertaining to the operations of medical services. Actively participates and takes a leadership role in quality assurance and utilization review.

Primary Duties and Responsibilities

  • Ensures that the daily operating medical activities are appropriate by assigning and overseeing employee workload, to maximize staff productivity which promotes a seamless patient flow process which will maximize resources and improve patient satisfaction. In compliance with CHN’s clinical standards for quality care and responsiveness to patient’s needs.
  • Maintains effective and responsive staffing for medical operations. Approves staff Flexible Time Off (FTO), gives advance notification to the Director of Operations regarding potential staffing issues.
  • Assumes accountability for adequate scheduling of staff based on patient volume, provider availability, and operational needs. Makes staffing recommendations to the Director of Operations, as needed.
  • In conjunction with the Director of Operations monitors daily patient volumes and associated workload (productivity, staffing ratios, patient flow, etc.) to meet required organizational productivity ratios.
  • Ensures compliance with regulatory and policy issues related to facilities, clinical practice, risk management.
  • Maintains a current inventory of clinical supplies.
  • Works cooperatively with Front Office Supervisor to ensure accuracy of provider templates and provides the clinical staff with timely updates regarding provider template and schedule changes. In conjunction with Front Office Supervisor, completes weekly staffing schedule.
  • Assists with resolution of patient complaints and resolution of patient concerns for the medical practice.
  • Assists with the development, implementation, and documentation of medical departmental policies and procedures, and ensures that clinical activities are in accordance with these policies.
  • Has a complete understanding of the organization nursing, medical assistant, registration, discharge, referral, and medical records process, to oversee the comprehensive training of clinical employees.
  • Provides nursing coverage as needed to support efficient medical operations.
  • Works closely and communicates with the Director of Operations on all activities related to medical services and promptly gives notification of problems or potential problems.
  • Ensures complete and effective orientation including performance evaluation of introductory/probationary period.
  • Coordinates and facilitates clinical staff meetings.
  • Act as a facilitator to identify opportunities to improve CHN. Responds to and documents various issues including personnel complaints, staff issues, conflicts, and opportunities for improvement. Supports and follows the CHN Employee Concern and Resolution Process.
  • Participates in Community Health Net committees, as required, and disseminates information appropriately.
  • Perform other job duties as assigned.

Infection Control Duties and Responsibilities

  • Conducts an annual facility infection risk assessment.
  • Develops a plan for infection surveillance, prevention, and control activities base on the facility risk assessment.
  • Maintains an infection surveillance, prevention, and control program based on the organization’s risk assessment and plan, in accordance with state, federal, and other reporting requirements.
  • Organizes and maintains surveillance data and records.
  • Reports surveillance data, findings, and analysis of the appropriate committees.
  • Develops evidence-based infection prevention and control policies and protocols in accordance with regulatory, government, and accrediting/licensing agency requirements and recommendations of relevant professional organizations.
  • Updates policies and protocols as needed to maintain compliance with regulations, standards, and evidence-based guidelines.
  • Monitors and evaluates the efficacy of infection prevention and control strategies.
  • Participates in required annual education programs.
  • Provides education on hand hygiene, aseptic technique, infection prevention and control strategies, antiseptics and disinfectants, specific infectious diseases, and other related health care issues.
  • Develops or obtains educational materials and programs, as needed, for personnel, patients, and visitors.

Requirements/Qualifications

Current Pennsylvania Registered Nurse (RN) or Licensed Practical Nurse (LPN) and CPR certification required. A minimum of five (5) years of nursing experience and three (3) to five (5) years supervisory experience in a medical doctor’s office or equivalent experience required. Strong PC knowledge and keyboarding skills are also required. Proven excellence in interpersonal relations, problem-solving, organizational skills, and ability to follow through to completion required. As well as, an ability to handle numerous interruptions and multi-task required. Must be able to work flexible hours, fill-in for absent staff, and work weekends. A positive attitude and ability to communicate professionally and courteously with the public. Proven ability to work effectively with providers, Administrative Staff, and other community medical providers required. Proven organizational and time management skills required. Confidentiality in handling patient and organizational information mandatory. Ability to step into all clinical/nonclinical positions within the office and function during times of staffing needs. Experience with NextGen or other EHR systems is preferred.

POSITION TITLE: Pharmacy Technician

REPORTS TO: Director of Pharmacy

STATUS OF POSITION: Full-time

SUMMARY
Under the direction of a licensed Pharmacist fills medication orders and other health care products for Community Health Net (CHN) patients. Performs routine tasks to help prepare prescribed medication, such as counting tablets and labeling bottles. Also performs administrative duties, such as answering phones, stocking shelves, and operating cash registers. Refers any questions regarding prescriptions, drug information, or health matters to a Pharmacist.

Essential Duties and Responsibilities

  • Receive and process written, electronic, or faxed prescription orders and verify that information is complete and accurate.
  • Accurately retrieve demographic, insurance, medication allergy and CHN program eligibility information from Community Health Net’s Electronic Health Record (EHR) as needed.
  • Maintain proper storage and security conditions for drugs.
  • Fill bottles/vials with prescribed medications and affix labels.
  • Under the guidance of a Registered Pharmacist, reconstitute pharmaceutical products according to written prescriptions.
  • File and retrieve hard-copy prescriptions that have been processed.
  • Clean and help maintain equipment and work areas.
  • Establish and maintain patient profiles, including lists of medications taken by individual patients.
  • Receive and store incoming supplies, verify quantities against invoices, and inform supervisors of stock needs and shortages as directed.
  • Answer telephones and respond to questions or requests.
  • Operate cash registers to accept payment from customers.
  • Assist customers by answering simple questions, locating items or referring them to the pharmacist for medication information or counseling.
