POSITION TITLE: Behavioral Health Consultant
REPORTS TO: Behavioral Health Coordinator

Summary

Provides consultation to primary care team, including the patients’ medical, dental, and psychiatric providers. Responsible for supporting and coordinating the mental and physical health care of Community Health Net patients’ medical provider and consulting psychiatrist.

Essential Duties and Responsibilities

  • Perform Behavioral Health consultation for primary care providers by providing brief behavioral interventions using evidence-based techniques.
  • Conduct screenings and assessment of Behavioral Health/Substance Abuse patients.
  • Coordinate patient care and refer to external providers for additional treatment as needed.
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.
  • Collaborate with patients’ medical provider to develop individualized treatment plans.
  • Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider.
  • Triage patients to determine where their needs can be best met.
  • Document and update patient’s chart in electronic health record as per Community Health Net Clinical Policies and Procedures.
  • Meet productivity and quality standards set forth by Community Health Net.
  • Performs other job duties as assigned.

Education/Development

Required

  • Master’s Degree in Healthcare, Human Services, or related field.
  • Valid and current Pennsylvania (PA) License as a: LCSW, LPC, and LMFT
  • Minimum of one (1) year of work experience in a behavioral health or social service setting.

Desired

  • Experience working with underserved patient populations and safety net providers.
  • Experience working with Pennsylvania and Federal programs.

Position Summary

Community Health Workers (CHW) are frontline public health workers who have a close understanding of the community we serve. CHW will be responsible for fostering trusting relationships between health/social services and the community, thereby serving as intermediaries to patients and their families, helping to navigate and access community services, other resources, and adopt healthy behaviors.

SUMMARY

Responsible for the delivery of quality healthcare for clients of Community Health Net. Primary duties include providing a complete health history and accurately determining client’s medical condition. In cooperation with the medical provider, promotes, maintains, and restores the well- being of the patient in a professional manner. Coordinates referrals to other health and social agencies; establishing and maintaining cooperative relationships with clients, families, providers, and coworkers. Maintains an awareness of medical equipment and supplies in a cost effective and safe manner. All nursing duties assigned are pending training and successfully passing competencies. Supports office activities of registration, discharge, medical records, and scheduling, as needed.

Essential Duties and Responsibilities

  • Assumes responsibility for acquiring and documenting a complete health history by utilizing interviewing and assessment skills.
  • Demonstrates a basic knowledge of pathology, treatment, and implications for patient education.
  • Demonstrates ability to set priorities based on nursing assessment and displays strong organizational skills.
  • In cooperation with the Provider, promotes, maintains and/or restores the well being of the client in professional manner.

  • Adheres to Community Health Net, JCAHO, and OSHA policies related to infection control practices.
  • Collects specimens to assist in diagnosis determination, including but not limited to, venous blood draw, sputum, urine, stool, etc.
  • Maintain appropriate documentation, quality control testing, and practices in compliance with the Community Health Net Health Care Plan.

Medical Director Position Mission Statement: To improve the quality of life in our region by being an effective leader of the Community Health Net clinicians to ensure high quality and productive medical care is being provided to Community Health Net patients.

Summary

The Medical Director serves as the senior clinical advisor to Community Health Net management. This position represents the interests of the practicing clinicians to Administration and the Board of Directors. Simultaneously, the Medical Director serves to define and explain corporate goals and directives to the medical staff. The Medical Director must also be involved in efforts to ensure that all clinicians comply with all regulatory and legal requirements. Must also direct efforts at maintaining and improving the quality of care provided to Community Health Net patient populations.

Duties and Responsibilities

Administrative

  • Lead responsibility in all clinical medical services.
  • Functions as medical advocate for the Chief Executive Officer to ensure smooth operation of the primary care facility.
  • Supervises review and auditing of medical records and peer review.
  • Monitor provider compliance.
  • Plans and holds meetings with Provider and clinical staff, to assume lead responsibility in the generation and institution of operational medical policies and protocols.
  • Expands the Community Health Net utilization review capabilities, by working with the quality initiatives of the organization, chart audit committee, fiscal department, and other Community Health Net groups.
  • Provides medical oversight of clinical programs, such as the Diabetes Collaborative.
  • Works cooperatively with the Finance Department on relevant fiscal issues to ensure and track medical clinic productivity measures.
  • Assumes lead role in Provider training on chart documentation and patient visit auditing, to maximize the accuracy of the notes, increase generation of revenue, and meet Quality Improvement and Corporate Compliance standards. This will be accomplished with the support appropriate Community Health Net personnel and Board of Directors directed committees.
  • Generates the on-call and rounding schedule for the primary care practice.
  • Coordinate Provider template to include vacation, sick and continuing medical education hours. Ensuring proper Provider coverage to meet organizational patient satisfaction and productivity requirements.

Medical Services

  • Works to improve productivity of medical providers by developing and implementing policies and protocols that promote a professional and efficient clinical practice.
  • Coordinate training and continuing medical education, for medical staff, on a variety of medical specialties.
  • Develops, reviews, and updates medical clinical protocols and policies for Community Health Net.

Staff Supervision

  • Completes the required performance appraisals for medical providers.
  • Communicates all pertinent organizational information to Provider staff.
  • Develops staff education and training programs for medical provider staff.
  • Leads in the development of structured patient education programs.

Requirements/Qualifications

Graduation from an accredited medical school and a current license to practice medicine in the Commonwealth of Pennsylvania is required. Board certification is required. Minimum five (5) years’ experience as practicing physician required. Minimum of 3 years administrative experience in a clinical healthcare related field. Meets and maintains credentialing and privileging criteria as outlined in the credentialing and privileging policy.

Summary

Under the direction of a licensed Pharmacist fills medication orders and other health care products for
Community Health Net (CHN) patients. Performs routine tasks to help prepare prescribed medication, such as
counting tablets and labeling bottles. Also performs administrative duties, such as answering phones, stocking
shelves, and operating cash registers. Refers any questions regarding prescriptions, drug information, or health
matters to a Pharmacist.

Essential Duties and Responsibilities

  • Receive and process written, electronic, or faxed prescription orders and verify that information is complete and accurate.
  • Accurately retrieve demographic, insurance, medication allergy and CHN program eligibility information from Community Health Net’s Electronic Health Record (EHR) as needed.
  • Maintain proper storage and security conditions for drugs.
  • Fill bottles/vials with prescribed medications and affix labels.
  • Under the guidance of a Registered Pharmacist, reconstitute pharmaceutical products according to written prescriptions.
  • File and retrieve hard-copy prescriptions that have been processed.
  • Clean and help maintain equipment and work areas.
  • Establish and maintain patient profiles, including lists of medications taken by individual patients.
  • Receive and store incoming supplies, verify quantities against invoices, and inform supervisors of stock needs and shortages as directed.
  • Answer telephones and responds to questions or requests.
  • Operate cash registers to accept payment from customers.
  • Assist customers by answering simple questions, locating items or referring them to the pharmacist for medication information or counseling.

The Scheduling Liaison will be responsible for scheduling patient appointments and communicating with patients prior to and following receipt of outpatient services. This individual will maintain patient demographics and interact with patients and others (on the telephone and in person) in a courteous and respectful manner.

  • Assist patients with patient form completion in accordance to Community Health Net policy and procedure.
  • Contact all patients regarding missed appointments including contacting all patients that were “No Shows” (follow complete process).
  • Confirm all cancelled appts are rescheduled in Practice Management System.
  • Receive all patient phone calls regarding appointments such as scheduling, rescheduling, etc.
  • Act as the liaison between patients and clinical staff during patient hours.
  • Documents in a professional manner all interactions with patients in the Electronic Health Record (EHR) system.

Qualifications/Requirements
